Stonecrest Living is a place that welcomes and honors people’s collective life experiences and challenges; this in turn informs our model of care focused on the values of love, person-centeredness, cultural intelligence and collaboration. Caregiving is not just an active practice but one that offers intentionality; it is interspersed through routines, activities rooted in personal interests and goals, nutritious meals prepared with care, life skills are practiced, and purpose is engaged.
We weave each individual’s culture, language, religion, and family involvement into decision-making. Drawing on the wisdom and strength of our African values of community, respect, and service to others, we believe every interaction with an individual should be one in which they feel that their life always holds meaning, even in moments of great instability.

The Stonecrest Living Collective Caregiving model is loving, person-centered, culturally intelligent and highly collaborative, where belonging is interspersed through routines, activities are rooted in personal interests and goals, nutritious meals are prepared, skills are practiced and living is dignified. We offer comprehensive, holistic support by navigating each person’s needs with responsiveness, curiosity, and humility. Through warm, relational and fully supported transitions, we help individuals reconnect with their strengths, regain a sense of control and move forward with greater clarity.
Our Reach
Stonecrest Living is a proud provider in over 8 counties in Minnesota, with 18 Community Residential Settings sites across the state. Additionally, at our main campus, the Landings of Minnetonka, we offer a 90-unit assisted living community.
